In accordance with the Kentucky Public Service Commission (PSC), all farm tap owners need to submit an Application for Gas Service. Please find enclosed the link to downrload the form.
The completed form can be mailed to the address at the top of the application or emailed to [email protected].
Please note that failure to submit the completed application could result in an interruption to natural gas service.
